LSS Hints: Finding Coumadin Flow Sheet

To find the coumadin flowsheet in a patient's chart, use the Documents function in LSS. Basically, you need to open the documentation function, and then open up the the new documents list, and then immediately cancel.  This will take you to a list of encounters for that patient which can be filtered into active documents.  The coumadin flowsheet will be one of the active documents (i.e. not signed documents).

Here's how to do this in detail, for charts with no active document already started for the current encounter:

  1. Open the appropriate patient's chart, and enter the documentation tool.  If there is no document already created, you will be presented with a list of document template types.  Do not select one of the template types.  Instead, click on the Cancel footer button to remove this list.
  2. You will see a list of clinic notes divided into subtypes.  Select the active documents by clicking on the Active document toggle button.  This should be the default.
  3. Click on the All Documents toggle button to select this function.  This should also be the default selection in this screen.  The coumadin flowsheet should be in the History subtype.  Double click on the line entry for coumadin flowsheet to open up the flowsheet document.
  4. Active documents are documents that have not been signed, and are still in draft form.  Most nurses notes are in draft form and will be visible by selecting the Active toggle button.  If you want to see all the notes, select the Both toggle button.  Select the Inactive toggle button to select signed documents.  Because the coumadin flow sheet is not ever signed, it should remain an active document.
Here is how to find the coumadin flowsheet in an encounter with a document already started:

  1. Enter the documentation tool for the chart selected.
  2. If there already is a document started for the encounter, don't edit the document.  Click on the Enter New footer button to get to the document template list. Don't select a document template.
  3. Immediately click on the Cancel footer button to remove the template list.  You should now see a list of clinic notes divided into subtypes, and the coumadin flowsheet should be easier to find.  Select the Active and All Documents toggle buttons to display a filtered list of notes, among which should be the coumadin flowsheet.
